A not-for-profit healthcare system is in need of a Remote Care Transitions Navigator in Springfield.
Individual must be able to fulfill the following responsibilities:
- Maintain ongoing contact with health care organizations and medical community
- Facilitate home care services and streamlines the transition of patient care between facilities and care providers
Skills and Requirements Include:
- Graduate of an approved License Practical Nursing program
- Minimum one year experience as professional nurse within the last three years
- Comfortable working remotely
