Remote Case Management Nurse

Job Summary

To support the Case Management team, the full-time Remote Case Management Nurse will educate members on health outcomes, assist with care transitions, and collaborate with support teams to ensure exceptional care delivery.

Key responsibilities:

• Assist in the coordination of care across various settings including inpatient, outpatient, and home care
• Develop care plans for members facing health challenges through proactive outreach
• Communicate with members to provide education on health conditions, medications, and procedures

Required qualifications:

• Active, unrestricted RN licensure from the United States or a compact multistate unrestricted RN license
• Ability to obtain additional state licenses as needed
• Minimum of 2 years of clinical experience in payer, hospital, outpatient, or community-based care management
• At least 1 year of experience in Care Coordination and Navigation
