A healthcare management company is seeking a Remote Case Manager in Bend.

Core Responsibilities of this position include:

• Ensuring quality driven outcomes through best practice and motivational interviewing
• Determining the needs of the member
• Coordinating services to ensure that needs are met in an individualized manner

Qualifications for this position include:

• Must have valid driver's license and private transportation
• Must have demonstrated experience working with severely and mentally ill population
• Must have personal cell phone and home internet connection
• Working knowledge of Federal, State and private health related activities
• At least 5 years clinical experience
• At least three years field based or telephonic case management
