A healthcare management company is seeking a Remote Case Manager in Bend.
Core Responsibilities of this position include:
- Ensuring quality driven outcomes through best practice and motivational interviewing
- Determining the needs of the member
- Coordinating services to ensure that needs are met in an individualized manner
Qualifications for this position include:
- Must have valid driver's license and private transportation
- Must have demonstrated experience working with severely and mentally ill population
- Must have personal cell phone and home internet connection
- Working knowledge of Federal, State and private health related activities
- At least 5 years clinical experience
- At least three years field based or telephonic case management
