Remote Case Manager RN

Job Summary

Working remotely on a full-time basis, the Remote Case Manager RN will manage comprehensive case management services for patients with chronic or catastrophic illnesses, ensuring high-quality, cost-effective healthcare while collaborating with healthcare teams and advocating for patient needs.

Key responsibilities

• Conduct assessments to identify patients' medical and psychosocial needs and develop individualized care plans
• Facilitate communication among patients, families, and healthcare providers to ensure coordinated care and adherence to treatment plans
• Maintain accurate and timely documentation of case management activities and evaluate the effectiveness of care plans regularly

Required qualifications

• Registered Nurse with a minimum of 5 years of clinical experience
• Active, unrestricted multi-state nursing license
• Preferred credentials such as CCM, CRRN, or OCN
• Recent clinical experience in a relevant healthcare setting
• Strong organizational and time management skills, with the ability to work independently
