Job Summary
A healthcare company has an open position for a Remote Case Manager RN.
Candidates will be responsible for the following:
- Assessing, facilitating, planning and coordinating an integrated delivery of care across the continuum
- Ensuring that patients progress toward desired outcomes with quality care that is medically appropriate
- Completing comprehensive assessments of members per regulated timelines and determining who may qualify for case management
Position Requirements Include:
- Having graduated from an Accredited School of Nursing
- Having 1-3 years in case management, disease management, managed care or medical or behavioral health settings
- Having an active, unrestricted State Registered Nursing (RN) license in good standing
- Having a valid driver's license with a good driving record and be able to drive within applicable state or locality
