Job Summary
A global healthcare company has an open position for a Remote Case Manager RN Registered Nurse.
Core Responsibilities of this position include:
- Making outbound calls and receiving inbound calls to assess members' current health status
- Providing patient education to assist with self-management
- Supporting members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
Applicants must meet the following qualifications:
- 3+ years of nursing experience in a hospital, acute care or direct care setting
- Current, unrestricted Compact RN license in the state of employment
- Nursing diploma OR ASN
- Experience or exposure to discharge planning
- Must be able to type and have the ability to navigate a Windows based environment
