Remote Certified Medical Assistant Care Coordinator

Job Summary

A developer of healthcare management software is in need of a Remote Certified Medical Assistant Care Coordinator.

Must be able to:

• Conduct monthly phone calls with patients, practices, pharmacies, etc
• Create and/or revise a personalized care plan for each patient
• Work to identify social determinants of health

Must meet the following requirements for consideration:

• 1-2 years healthcare experience as a Certified/Registered Medical Assistant with active accreditation
• Familiar with Microsoft Office, GoogleSuite products and comfortable learning new softwares quickly
• Exceptional business writing skills, including accurate spelling and grammar
• Have a home office set up that will allow you to speak privately and maintain confidentiality
