Remote Certified Medical Assistant Care Coordinator

Job Summary

A healthcare company has a current position open for a Remote Certified Medical Assistant Care Coordinator.

Individual must be able to fulfill the following responsibilities:

• Conduct monthly phone calls with patients, practices, pharmacies, etc
• Create and/or revise a personalized care plan for each patient you call on, in coordination with the practices and providers you support
• Work to identify social determinants of health, gaps in care, and eligibility for assistance or other referral services

Qualifications for this position include:

• 1-2 years healthcare experience as a Certified/Registered Medical Assistant with active accreditation
• At least 6 months experience working remotely from home
• Technology experience
• Be highly professional on the phone
• Be able to maintain a compassionate and engaging tone
• Possess quick thinking skills to respond live to patient questions and needs
