Job Summary

A healthcare system is searching for a person to fill their position for a Remote Childrens Comprehensive Care Clinic RN Case Manager.

Individual must be able to fulfill the following responsibilities:

  • Advocate for resources and removal of barriers
  • Monitor patient care through assessments
  • Coordinate the overall interdisciplinary plan of care

Applicants must meet the following qualifications:

  • Licensed Registered Nurse credentialed from the Texas Board of Nursing required
  • Diploma from an accredited school/college of nursing required
  • 2 years of Case Management experience preferred
