Remote Chronic Care LVN

Job Summary

To support patient care initiatives, the full-time Remote Chronic Care LVN will conduct high-volume outbound calls to assess patient needs, develop care plans, and provide medical device troubleshooting, all while working in a remote clinical environment.

Key Responsibilities

• Manage inbound and outbound telephonic patient interactions in a high-volume clinical call center
• Conduct comprehensive assessment calls and document care plan interactions in real time
• Collaborate with healthcare providers to ensure effective patient care coordination

Required Qualifications

• Active LPN/LVN Compact License required
• Strong proficiency with computers, EMRs, and telehealth tools
• Passion for patient care and teaching, with the ability to relate with empathy
• At least 1 year of nursing experience preferred, particularly in RPM, telehealth, or chronic care
• Previous experience in a call center or high-volume phone-based role is a plus
