Job Summary
A digital health company has a current position open for a Remote Chronic Care Management Coordinator.
Core Responsibilities Include:
- Meeting production goals based on hours
- Reviewing individual care plans and making monthly phone calls to patients
- Managing the flow of information to and from provider’s office
Must meet the following requirements for consideration:
- Medical Assistant certification or higher (LPN, RN, BSN, etc)
- Strong verbal and written proficiency in English
- Minimum 1-year experience in home-care or clinical setting
- Minimum 1-year experience in Chronic Care Management
- Knowledge of medical terminology
- HIPAA compliant area in home to conduct Chronic Care Management duties
