Job Summary
A health insurance company is filling a position for a Remote Chronic Kidney Disease RN Nurse Case Mgr II.
Candidates will be responsible for the following:
- Ensuring member access to services appropriate to their health needs
- Conducting assessments to identify individual needs
- Coordinating internal and external resources to meet identified needs
Position Requirements Include:
- BA/BS, 5 years of clinical experience, or any appropriate combination of education and experience
- A current, unrestricted RN license in Ohio. Multi-state licensure may be needed
- Ability to obtain a multi-state RN license if needed
- Comfort working with web-based applications using multiple screens
