Job Summary
A healthcare company is in need of a Remote Clinical Care Coordinator.
Candidates will be responsible for the following:
- Assessing, planning and implementing care strategies
- Identifying and initiating referrals for social service programs
- Managing the care plan throughout the continuum of care
Skills and Requirements Include:
- Travel 50 % of the Time
- Background in managing populations with complex medical or behavioral needs
- 5+ years of clinical, Employment, or case management experience
- 5+ years of leadership or management experience
- 2+ years of experience in long-term care, home health, hospice, or related
- 5+ years of experience working with MS Word, Excel and Outlook
