Job Summary

A staffing firm is seeking a Remote Clinical Care Manager in Cincinnati.

Core Responsibilities of this position include:

• Providing case management services to members with chronic or complex conditions
• Conducting assessment of needs by collecting in-depth information
• Identifying, assessing and managing members per established criteria

Required Skills:

• Valid driver's license and ability to conduct face to face or home visits as required
• 1-2 years of current or recent clinical case management experience
• Bachelor's degree in nursing, social work or health education from an accredited school
• Valid unrestricted OH State RN or LSW license in good standing
• Ability to perform ongoing monitoring of the plan of care to evaluate effectiveness
• Ability to document care plan progress and modifies as appropriate to reach optimal outcome
