A healthcare company has a current position open for a Remote Clinical Integration Nurse Case Manager in Worcester.

Individual must be able to fulfill the following responsibilities:

  • Telephonically assess and case manage a member panel
  • Perform medication reconciliations
  • Utilize clinical judgment and nursing assessment skills

Skills and Requirements Include:

  • May conduct in home face to face visits
  • Active, unrestricted license as a Registered Nurse in Massachusetts
  • Current Driver’s license and a vehicle to be used for home visits
  • Satisfactory Criminal Offender Record Information results
  • 1+ years as a Registered Nurse managing chronically ill members or experience in a coordinated care program
  • Understanding of Hospitalization experiences and the impacts and needs after facility discharge
