Job Summary
A healthcare company is seeking a Remote Clinical Operations Care Coordinator.
Candidates will be responsible for the following:
- Providing relevant self-management education and supporting patients with chronic illnesses
- Assisting with data collection and generating patient summary reports
- Addressing gaps in care and assisting patients in setting up healthcare appointments to close care gaps
Skills and Requirements Include:
- Must have a minimum of 2 years of direct patient care addressing complex/chronic health conditions
- Associate or bachelor's degree in healthcare related field or equivalent professional experience, required
- Licensed Practical Nurse, Medical Assistant, or other health related certification
- Proficient in utilizing Microsoft Outlook, Teams, and Point
- Competency in utilizing technology to communicate and document clinical activities
- Care coordination, navigation, advocacy, or care management experience ideal
