Job Summary

A home health care agency is in need of a Remote Clinical Pharmacist in Minnetonka.

Core Responsibilities Include:

  • Communicating with Recover Health nurses to avoid communication gaps
  • Reviewing therapy plans with physicians as needed
  • Evaluateing medication profiles to determine if changes are warranted

Skills and Requirements Include:

  • Valid and unrestricted Wisconsin pharmacy license
  • Ability to obtain NPI and other required credentials
  • Graduated from a U.S. school of pharmacy after May 1996 with a PharmD
  • Have no restrictions on ability to participate in private, federal or state insurance programs
  • Must have own internet connection and computer
