Job Summary

A health services company is filling a position for a Remote Clinical Practice Performance Coordinator in Denver Metro Area.

Individual must be able to fulfill the following responsibilities:

  • Conduct telephonic patient outreach to patients with chronic conditions and/or preventative care needs
  • Provide patient education and guidance on disease management and care coordination
  • Build and maintain relationships with independent practices (IPA)

Required Skills:

  • High School Diploma, GED or equivalent
  • Medical Assistant or Licensed Practical Nurse (LPN) experienced
  • Experience with HEDIS Quality
