Job Summary

A healthcare company has a current position open for a Remote Collaborative Care Case Manager.

Candidates will be responsible for the following:

  • Efficiently accepting patient handoffs from an Intake Care Manager and seamlessly continue care
  • Developing rapport with patients to create a safe therapeutic environment
  • Encouraging patients to express feelings and discuss what is happening in their lives
  • Master's Degree in psychology, social work, or another behavioral health discipline from an accredited college or university
  • Proficient in Microsoft Word, Google Suite, and Excel
  • The strong data entry experience
  • Work experience with an Electronic Medical Records
