Remote Collaborative Care Clinician

Job Summary

A healthcare company has a current position open for a Remote Collaborative Care Clinician.

Candidates will be responsible for the following:

• Screening and assessing patients for common mental health and substance abuse disorders
• Establishing a mutually agreeable care management plan
• Monitoring and engaging patients by phone for changes

Qualifications Include:

• Active mental health professional license
• At least one year of post-graduate clinical work experience
• Proficiency in evidence-based treatment approaches
• Experienced utilizing  behavioral health tools
• Familiarity with brief, structured intervention techniques using evidenced-based techniques
• Highly proficient in using many different platforms at one time, including EMRs
