A healthcare company has an open position for a Remote Community Based Case Manager in the Logan Area.
Individual must be able to fulfill the following responsibilities:
- Managing a caseload of Ohio Home Care individuals covering several counties
- Assessing, authorizing and providing ongoing case management
Position Requirements Include:
- This is a field based position
- 2 months experience in home and community based services within the past 5 years
- Active license as RN, LSW, or LISW
