A healthcare company is filling a position for a Remote Community Health Plan Worker in Grand Rapids.
Core Responsibilities of this position include:
- Assessing if eligible for MiBridges and SNAP and assist with application
- Conducting member outreach
- Assisting with reporting requirements once a month per member 80 hours of work or activities like job search
Skills and Requirements Include:
- Travel 25 % of the time
- Previous experience working with HMP / Medicaid population
- Experience with Michigan Medicaid policies and procedures
- High School Diploma / GED (or higher)
- Familiarity with the resources available within the community
- Knowledge of culture and values in the local community
