Job Summary
A global healthcare company is in need of a Remote Community Health Worker in Northwest Kansas.
Must be able to:
- Plan and implement care strategies that are individualized by patient
- Work with the clinical coordinators to facilitate referrals for social service programs
- Advocate for patients and families to ensure the patient’s needs/choices are fully met
Qualifications for this position include:
- Spending about 50% of your time in the field visiting Medicaid members
- Being flexible, adaptable and, above all, patient in all types of situations
- 3+ years of experience working within the community health setting
- Experience in long-term care, home health, public health, assisted living, or related facility
- Experience working with MS Word, Excel and Outlook
- High School diploma required
