Job Summary
A health care system company is in need of a Remote Community Resource Specialist Patient Navigator.
Must be able to:
- Contribute to quality improvement and process design of patient navigation models
- Gather and manage quantitative and qualitative patient data
- Design, implement, and utilize screening tools and program resources
Qualifications for this position include:
- Willingness to travel to primary care practice sites and relevant specialty service sites
- Ability to interview patients using interactive techniques
- High School Diploma or GED Required
- Customer service experience within healthcare services, community and/or multicultural settings
- Proficient in Microsoft Applications including MS Office
