Job Summary

A healthcare company is in need of a Remote Compact State Telephonic Registered Nurse Case Manager.

Must be able to:

  • Make outbound calls to assess members' current health status
  • Identify gaps or barriers in treatment plans
  • Provide patient education to assist with self-management

Required Skills:

  • Current, unrestricted RN license in the state of Residence
  • Multiple state licensure (in addition to Compact License if applicable) or ability to obtain
  • 3+ years of experience as a Registered Nurse in a hospital, acute care or direct care setting
  • Willing and able to obtain CCM within 30 months of employment
  • Intermediate proficiency level of experience with MS Office applications
  • Dedicated, distraction-free workspace in home for home office
