Job Summary

A healthcare services organization is searching for a person to fill their position for a Remote Complex Care Transitions Advanced Practice Provider.

Candidates will be responsible for the following:

• Applying evidence-based guidelines and protocols to manage various conditions
• Incorporating a range of remote patient monitoring data to inform clinical decision-making
• Evaluating and treating patients engaged in the program at least weekly using telemedicine

Qualifications Include:

• Current, unrestricted license in Arizona and Georgia
• Graduate of an accredited Nurse Practitioner program with a current NP certification
• Three years or more of clinical experience
• Comfortable treating complex adults aged 65+
• Proficient in review data and documenting in electronic health records
