Job Summary
A health insurance company is searching for a person to fill their position for a Remote Discharge Care Manager Registered Nurse.
Must be able to:
- Evaluate member discharge needs including delays in care and readmission prevention plan
- Collaboration with providers and members to coordinate care post discharge
- Participate in rounds with the Medical Director to discuss cases as needed
Qualifications for this position include:
- A current, unrestricted RN license
- A Bachelor’s of Science in Nursing
- 3 or more years total experience in the inpatient acute setting
- 3 years’ experience of discharge planning
- Proficiency in typing skills and software applications
- Experience working with multiple health insurance products
