Remote Discharge Care Manager RN

Job Summary

A company is looking for a Discharge Care Management Nurse RN to work remotely in the Central or Eastern Time Zone.

Key Responsibilities

• Evaluate member discharge needs and develop readmission prevention plans
• Coordinate care post-discharge by collaborating with providers and members
• Document discharge activities and participate in team meetings and discussions

Required Qualifications

• Bachelor of Science in Nursing
• Current, unrestricted Compact RN license in the State of residence
• 5+ years of recent experience in an inpatient acute setting
• 2+ years of experience in discharge planning
• Experience with multiple health insurance products and regulatory requirements
