Job Summary

A health insurance company needs applicants for an opening for a Remote Disease Management Nurse I in Asheville.

Core Responsibilities of this position include:

  • Conducting behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs
  • Identifying and/or coordinating specific health coaching plan needs to address objectives and goals identified during assessments
  • Interfacing with provider and other health professionals to coordinate health coaching plan for the member

Required Skills:

  • Requires an AS/BS in nursing
  • 2 years of condition specific clinical or home health/discharge planning experience
  • OR any combination of adequate education & experience
  • Current unrestricted RN license in applicable state(s) required
