A healthcare technology company is filling a position for a Remote Disease Manager II.
Core Responsibilities of this position include:
- Developing future-oriented goals and health actions that assist members
- Advocating for members and families
- Evaluating referrals for potential enrollment of members
Skills and Requirements Include:
- Possession of a valid driver’s license and the ability to travel
- At least three (3) years’ recent/related experience
- Registered Nurse with active licensure in home state
- Knowledge of NCQA standards for case management organizations
- Knowledge of Care Continuum Alliance standards for chronic condition management
- Knowledge of clinical and managed care principles and operations
