Job Summary
A healthcare company has a current position open for a Remote Enhanced Community Care Management Social Worker.
Individual must be able to fulfill the following responsibilities:
- Engage the patient/family/significant other and interdisciplinary team
- Provide crisis intervention/counseling
- Complete and document psychosocial assessments, intervention, and coordination of care to identified populations
Qualifications for this position include:
- Actively participates in those areas of expertise to further enhance the role of social work within the hospital
- Master's in Social Work
- Older Adults Protective Services Act (OAPSA) clearance may be required depending on location
