Remote Fertility Social Worker

Job Summary

A healthcare company is seeking a Remote Fertility Social Worker.

Candidates will be responsible for the following:

• Assess patient and family psychosocial needs and develop plan of care
• Link patients and family to appropriate community resources
• Document patient/family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations and progress in medical record

Required Skills:

• Masters in Social Work (MSW) with active and unrestricted social work license in the state of residence
• Training in the Medical and Psychological Aspects of Infertility
• years of clinical experience providing infertility counseling
• Proficient Computer skills in Microsoft Office such as Word, Outlook and Internet
• Dedicated, distraction-free area in home for home office
• Access to secure high speed internet via cable/DSL in home
