Job Summary

A healthcare company is seeking a Remote Field Assessment Registered Nurse in Brooklyn.

Candidates will be responsible for the following:

  • Performing the NYS UAS Assessment in the member’s home
  • Managing the care plan throughout the continuum of care
  • Communicating with all stakeholders

Qualifications Include:

  • Ability to travel as needed
  • 4+ years of experience working within the community health setting in a health care role
  • Experience in long-term care, home health, hospice, public health or assisted living
  • Current, unrestricted RN license for the state of New York
