A healthcare company has an open position for a Remote Field Care Manager Nurse in Birmingham.

Candidates will be responsible for the following:

  • Managing a member's physical, environmental, and psycho-social health issues
  • Identifying and resolving barriers that hinder effective care
  • Following established guidelines/procedures

Qualifications Include:

  • Valid driver's license, car insurance, and access to an automobile
  • Registered Nurse with 2 years of experience of in home case/care management
  • RN License with no restrictions in the state of Alabama
  • Experience working with the adult population
  • Knowledge of community health and social service agencies and additional community resources
  • Ability to work with minimal supervision within the role and scope
