Job Summary
A healthcare provider is searching for a person to fill their position for a Remote Field Case Manager in Richmond.
Individual must be able to fulfill the following responsibilities:
- Coordinate and facilitate quality, cost-effective care
- Provides coordination of care between the patient, family, physician, provider and care teams
Qualifications for this position include:
- Travel to dialysis centers in designated geographic area
- A.D.N degree from accredited school of nursing
- Current Registered Nurse (RN) license in the state of practice
- Continuing education credits maintained as required by state of practice
- Minimum of five (5) years’ experience in clinical nursing
- Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes
