Job Summary
A health insurance company is in need of a Remote Florida Licensed Medicare Registered Nurse Telephonic Care Manager.
Must be able to:
- Manage a member's physical, environmental and psycho-social health
- Identify and resolve barriers that hinder effective care
- Ensure patient is progressing towards desired outcomes
Qualifications for this position include:
- Minimum 3 years of clinical care experience
- RN license that includes FL, with no disciplinary action
- Experience with discharge planning and patient education
- Must have accessibility to high speed DSL or Cable modem
- Must have a home office with a door and a lock
- Ability to work independently under general instructions and with a team
