Job Summary
A healthcare provider is searching for a person to fill their position for a Remote Health After Care Coordinator.
Core Responsibilities of this position include:
- Following up on members who are identified by clinical operations as needing aftercare follow up
- Verifying compliance with initial follow-up appointments
- Providing assistance with referral to a provider if initial appointment is not attended
Applicants must meet the following qualifications:
- High school diploma or GED required
- 1 year experience in customer service and/or related field
- OR any combination of education and experience which would provide an equivalent background
