Job Summary

A healthcare provider is searching for a person to fill their position for a Remote Health After Care Coordinator.

Core Responsibilities of this position include:

• Following up on members who are identified by clinical operations as needing aftercare follow up
• Verifying compliance with initial follow-up appointments
• Providing assistance with referral to a provider if initial appointment is not attended

Applicants must meet the following qualifications:

• High school diploma or GED required
• 1 year experience in customer service and/or related field
• OR any combination of education and experience which would provide an equivalent background
