An insurance company has a current position open for a Remote Health and Social Services Clinical Coordinator in the Lockhart Area.
Core Responsibilities Include:
- Managing the care plan throughout the continuum of care as a single point of contact
- Identifying and initiating referrals for social service programs
- Assessing, planning and implementing care strategies
Position Requirements Include:
- Expect to spend about 50% of your time in the field
- Experience working with MS Word and Outlook
- RN current, unrestricted in the state of TX
- 4 years of experience working within the community health setting or in a health care role
