Job Summary
A healthcare provider has a current position open for a Remote Health and Social Services Clinical Transition Coordinator in Gainesville.
Core Responsibilities of this position include:
- Assessing NH LTC members for ability to transition into the community
- Identifying and initiating referrals for social service programs
- Managing the care plan throughout the continuum of care as a single point of contact
Qualifications for this position include:
- Travel Yes, 50 % of the Time
- Bachelor's degree
- 4+ years of experience working within the community health setting in a health care role
- Experience in long-term care, home health, hospice, public health or assisted living
- 1+ years of experience working with MS Word, Excel and Outlook
- All other requirements necessary for this position
