Job Summary
An insurance company needs applicants for an opening for a Remote Health and Social Services Coordinator in Kansas City.
Individual must be able to fulfill the following responsibilities:
- Identifying and initiating referrals for social service programs
- Assessing, planning and implementing care strategies
- Managing the care plan throughout the continuum of care as a single point of contact
Qualifications for this position include:
- Expect to spend about 50% of your time in the field visiting our members
- 4 years of experience working within the community health setting in a health care role
- Associates degree
- Experience in long-term care, home health, hospice, public health or assisted living
- 1 years of experience working with MS Word, Excel and Outlook
