Job Summary
A major healthcare company is filling a position for a Remote Health and Social Services Coordinator in the Southwest Kansas Area.
Individual must be able to fulfill the following responsibilities:
- Managing the care plan throughout the continuum of care as a single point of contact
- Identifying and initiating referrals for social service programs
- Advocating for patients and families as needed to ensure the patient's needs and choices are fully represented and supported
Applicants must meet the following qualifications:
- 50% of your time in the field visiting our members in their homes or in long-term care facilities
- 4 years of experience working within the community health setting in a health care role
- Associates degree
- Experience in long-term care, home health, hospice, public health or assisted living
- 1 years of experience working with MS Word, Excel and Outlook
