Job Summary

A healthcare company is searching for a person to fill their position for a Remote Health and Social Services RN Clinical Coordinator in Essex County.

Candidates will be responsible for the following:

  • Assessing, planning and implementing individualized care strategies
  • Identifying and initiating referrals for social service programs
  • Communicating with all stakeholders to ensure quality care and services are provided

Applicants must meet the following qualifications:

  • The ability to travel in assigned region to visit Medicaid members
  • Current, unrestricted RN license for the state of New Jersey
  • Associate’s degree or higher
  • 4+ years of experience working within the community health setting in a health care role
  • Experience in long-term care, home health, hospice, public health or assisted living
  • Proficiency with MS Word, Excel and Outlook
