Job Summary
A healthcare provider is in need of a Remote Health and Wellness Registered Nurse Care Manager in Ashland.
Candidates will be responsible for the following:
- Documenting activity and completing paperwork as required
- Identifying and coordinating clinical and functional supports needed for the member's health and safety
- Contacting and coordinating with acute and primary care providers
Applicants must meet the following qualifications:
- Travel to perform face-to-face health-related assessments
- Associates or better in Nursing
- Registered Nurse (RN)
- Must reside close to this area or plan on moving to this area to be considered
