Job Summary

A health insurance company is filling a position for a Remote Health Coach.

Core Responsibilities of this position include:

  • Coordinating specific health coaching as directed by nurse case manager
  • Conducting behavioral assessments to identify individual member knowledge, skills, and behavioral needs
  • Implementing coaching plan by using behavior change principles to identify member barriers

Applicants must meet the following qualifications:

  • BA/BS and minimum of 3 years of related experience in health education equivalent background
  • Certification, advanced certification, and/or license appropriate to field of specialty as required
  • Able to monitor and evaluate the interventions and modifies
