Remote Health Educator

Job Summary

An insurance company is seeking a Remote Health Educator.

Candidates will be responsible for the following:

• Implementing and/or coordinating coaching and/or care plans by educating members regarding health needs
• Identifying and/or coordinating specific health coaching plan needs to address objectives and goals
• Directing members to facilities, community agencies and appropriate provider/network

Qualifications Include:

• Requires BA/BS, 2 years of related experience or any combination of education and experience, which would provide an equivalent
• Prior experience in health coaching disease management in a call center environment
• Able to refer member to disease management or catastrophic case management
