Job Summary

A staffing agency is seeking a Remote Health Outreach Coordinator in Grand Junction.

Candidates will be responsible for the following:

  • Screening Medicaid members by phone for social needs
  • Providing information on community resources to members
  • Demonstrating multicultural sensitivity and effective communication skills

Required Skills:

  • Bachelor's in social work (or similar) or 4+ years of case management experience
  • 2 years of progressive related experience
  • Must have knowledge of poverty, interpersonal violence, and motivational interviewing
  • Experience with high volume customer service phone calls
  • Minimum 40+ WPM
  • Must be able to maintain confidentiality
