Job Summary
A privately-held population health management solutions company has an open position for a Remote Health Outreach Specialist.
Must be able to:
- Reach out to members and engage them in a discussion around case management program benefits and enrollment
- Maintain daily calling schedule
- Update and maintain notes in business operating system
Applicants must meet the following qualifications:
- High School degree or equivalent
- Two years minimum medical assistant experience or customer service experience in the medical field
- Strong professional verbal skills
- Experience in working with chronic disease states
- Ability to motivate people to participate in special programs
- Ability to effectively communicate with physician office staff
