Job Summary

A healthcare, life insurance, accident insurance, and Medicaid and Medicare service provider has a current position open for a Remote Healthcare Intake Lead Representative Care Coordination Lead.

Individual must be able to fulfill the following responsibilities:

• Establish a collaborative relationship with the client, physician(s), and other providers to coordinate care
• Document findings in a clear concise manner
• Maintain accurate record (system) of interventions
