Job Summary
A non profit Catholic healthcare system needs applicants for an opening for a Remote Healthcare Outpatient Care Coordinator Registered Nurse.
Core Responsibilities of this position include:
- Reviewing all patient data within the designated time frames to determine a plan of care
- Assessing, developing, implementing and monitoring a comprehensive plan of care through an interdisciplinary team process
- Assessing a patients functional level, their physical, cognitive, social, emotional status and their support system
Qualifications Include:
- Graduate of an accredited school of nursing
- Current Registered Nurse licensure in the state in which nurse will be practicing
- 3+ years of clinical nursing experience in an ambulatory or clinic setting
- Current Basic Life Support certification required at date of hire
- Skill and proficiency in applying highly technical principles, concepts and techniques
- Basic knowledge of statistics
