Job Summary
A healthcare company is filling a position for a Remote Healthcare Services Program Manager.
Core Responsibilities of this position include:
- Assessing, facilitating, planning and coordinating an integrated delivery of care
- Ensuring that patients progress toward desired outcomes with quality care
- Managing programs providing ongoing communication of goals
Applicants must meet the following qualifications:
- RN, Bachelor's or Master's in Nursing, Gerontology, Public Health, Social Work or related field
- 5+ years of managed healthcare experience
- 3+ years in utilization management, case management, care transition and/or disease management
- 2+ years of healthcare or health plan supervisory or managerial experience
- Experience working within applicable state, federal, and third party regulations
- Active, unrestricted RN or Social Work license in good standing
